After completing, at a minimum, the Pre-Application Phase prerequisite courses, an applicant must complete a Prerequisite & References Form. This form may be completed after November 1 and ideally no later than January 1st, as it is required prior to submission of the remainder of the application and provides reference forms to applicant-provided email addresses. This form may be submitted more than once if updates need to be made to prerequisite courses. Contact dieteticsms_advising@emich.edu with questions.
Reference providers can be professional or academic (not family members) and should be chosen based on their ability to assess multiple characteristics, including:
- Handling of stressful situations
- Independent work
- Organizational skills
- Leadership
- Flexibility/ability to adapt to new situations

- Ethical behavior/compassion for others
- Interpersonal skills
- Time management
- Oral communication

The form requires the applicant to list the course number, title, university, the year it was taken, credit hours, and the final grade for each completed prerequisite course, as well as, provide the contact information for two references. The listed references are e-mailed a reference form to complete and return.
The deadline for completing this form is January 1st.
A $25 Administration Fee is required. The fee may be paid by check or money order, addressed to: EMU Dietetics Program, 313 Marshall Bldg.,1215 N. Huron River Dr., Ypsilanti, MI 48197.
