Prerequisite and References Form

|
1) Dietetics Intent applicant will initiate the process by completing Reference Form and provide a non-refundable $35 processing fee prior to January 4, 2021. Please see the Program Application section below for additional information regarding the Processing Fee.
2) After completing, at a minimum, the Pre-Application Phase prerequisite courses, an applicant must complete a Reference Form. The References part of the form may be completed after November 1 and prior to January 4, 2021 Applicant will provide the contact information for two references. The listed references are e-mailed a reference form (from dietetics_application@emich.edu) to complete and return.
Reference providers can be professional or academic (not family members) and should be chosen based on their ability to assess multiple characteristics, including:
- Handling of stressful situations
- Independent work
- Organizational skills
- Leadership
- Flexibility/ability to adapt to new situations

- Emotional maturity
- Ethical behavior/compassion for others
- Interpersonal skills
- Time management
- Oral communication

- Written communication
- Effective problem solving/critical thinking
- Commitment
- Self-motivation
- Intellectual capacity/scholastic ability

3) Dietetics intent applicant will complete the Prerequisite Form once, with all courses and final grades reported, no later than January 4, 2021. The Prerequisite Form requires the applicant to list the course number, title, university, the year it was taken, credit hours, and the final grade for each completed prerequisite course. Dietetics intent applicant will provide all transcripts to the Office of Records and Registration (emich.edu/registrar) no later than January 8, 2021.
